Click here to pay the drop in fee, this will need to be done prior to appointment
One time payment for appointment, establishing care is not necessary, but that can be reviewed at the time of the appointment.
It is also to be noted that the document will be sent directly to your employer, so please have an email address at the time of the appointment for where the form will need to be sent.
Being a neutral party, I will ensure the employer gets it, in order to avoid any issues of lost documentation.
